What is Breast Augmentation?
Breast Augmentation enhances the size of breasts by using inflatable implants filled with saline or silicone (in certain cases).

How does Breast Augmentation work?
An incision is made in one of these locations: under the areola (the pink circle around the nipple), under the breast in the armpit area, in the belly button. After creating the incision, the surgeon lifts the breast tissue up to create a pocket either directly under the tissue or underneath the muscle, depending on the surgeon's judgment and the patient's physique.

Who is a good candidate for Breast Augmentation
The best candidate for breast enlargement is someone with realistic expectations about what the procedure will accomplish. Hoping for perfection will only bring disappointment. Enlarging your breasts will give you a more aesthetic appearance and may raise your self-confidence. The good news is you'll still be you, only better. Talk with your surgeon about your expectations and you can decide together how realistic they are.

What will be the results of Breast Augmentation?
Most implant manufacturers provide a lifetime warranty on their implants. Occasionally, implants may require removal or replacement. No surgeon can guarantee the outcome, but most will work with you to achieve the optimal results. If this means additional treatments in the treated area, many will usually do this for you without a surgical fee if they think it is medically safe.

How about safety & side effects Breast Augmentation?
Implants are not permanent; this means that it may sometimes be necessary to remove or replace them.
Some of the problems that may make it necessary to replace the implants include: deflation; the formation of scar tissue around the implant (capsular contracture), which may cause the breast to feel tight or hard; bleeding or infection.
An increase or decrease in sensitivity of nipples or breast skin, occasionally permanent, may result. After Breast Augmentation, mammography will require a special technique.
Temporary soreness, swelling, change in nipple sensation, bruising, and breast sensitivity to stimulation for a few weeks are the most common side effects. Make certain you understand all consent forms.

How painful is Breast Augmentation?
A very light general anesthesia is usually used during the surgery. There is only minimal pain that is mostly gone by the second or third day and is easily controlled by medications.

Testimonials form Breast Augmentation patients?
" I had been self-conscious about my small breasts ever since I was a teenager. From filling my bras with napkins to push-up bras to silicone inserts, I had tried them all. None looked or felt right. I asked all may girlfriends who had had the surgery (breast augmentation) and they were all happy they did it. The name of your surgeons kept coming up. I chose to get the surgery through my belly button. I am glad I did it. It is the best decision I ever made."

Pre-Breast Augmentation Instructions
Your surgeon will give you specific instructions on how to prepare for surgery, including guidelines on eating and drinking, smoking, and taking or avoiding vitamins, iron tablets and certain medications. If you develop a cold or an infection of any kind, especially a skin infection, your surgery may have to be postponed. Though rare, your doctor may recommend that you have blood drawn ahead of time in case it is needed during surgery. Also, while you are making preparations, be sure to arrange for someone to drive you home after the procedure and, if needed, to help you at home for a day or two. Special accommodations are available for patients who are coming from out of town.

Post-Breast Augmentation Instructions
After surgery, you will likely experience some fluid drainage from the incisions. Occasionally, a small drainage tube may be inserted beneath the skin for a couple of days to prevent fluid build-up. To control swelling and to help your skin better fit its new contours, you may be fitted with a snug elastic garment to wear over the treated area for a few weeks. Your doctor may also prescribe antibiotics to prevent infection. Do not expect to look or feel great right after surgery. Even though the newer techniques are believed to reduce certain post-operative discomforts, you may still experience some pain, burning, swelling, bleeding and temporary numbness. Pain can be controlled with medications prescribed by your surgeon, though you may still feel stiff and sore for a few days. It is normal to feel a bit anxious or depressed in the days or weeks following surgery. However, this feeling will subside as you begin to look and feel better.

Atricle-1-Breast Implant
A breast implant is a prosthesis used in cosmetic surgery to enhance the size and shape of one's breasts or to reconstruct the breast (for example, after a mastectomy). There are three types of breast implants:
- Saline-filled which have an external silicone shell and are filled with sterile saline liquid. These implants are currently the only ones available in the United States.
- Silicone gel-filled which have an external silicone shell and are filled with silicone gel. They are not currently available for implant in the United States, but future regulation may change this.
In the 60 countries outside the United States where silicone implants remain available, they are used in approximately 90% of implant operations. The FDA is conducting clinical trials of a new type of silicone gel implant filled with a new type of silicone called "cohesive silicone gel" that are believed to have decreased incidences of leakage.
- String implant, a third, much less common type of implant. This implant uses a polypropylene material as an implant. String implants are unique in that they cause the breast to perpetually expand after surgery, and are preferred by those women who choose to have the largest breasts possible.
They are not currently available for implant in the United States, but future regulation may change this.

History
Breast implants have been used at least since 1865 to augment the size of women's breasts. The earliest known implant occurred in Germany in which fat from a benign tumor was removed from a woman's back and implanted in her breast. In following years the medical community experimented with implants of various materials, most commonly paraffin. The first use of silicone as breast-implant material may have been by Japanese prostitutes in the period immediately following World War II.
Houston plastic surgeons Thomas Cronin and Frank Gerow developed the first silicone breast prosthesis with the Dow Corning Corporation in 1961 and the first woman was implanted in 1962. The implant was made of a silicone rubber envelope or sac filled with a thick, viscous silicone gel.

Risks and controversy
In the United States, implants from silicone gel were banned by the Food and Drug Administration because of growing concerns about the safety of such implants. More than one million women had availed themselves of the implants at the time of the ban, and the subsequent litigation led manufacturers to agree to a settlement of USD$4.25 billion. The degree of risk associated with silicone-gel breast implants is still a matter of debate within the scientific community.

Improperly performed breast implants
The health hazards of breast implants have been debated greatly in recent years. Some people believe that breast implants cause such illnesses as autoimmune disease, although both the AMA and FDA have found there to be no evidence of this.
However, documented problems with breast implants include rupture, deflation, infection, scarring and hardening of the implants.
According to the American Society of Plastic Surgeons, breast augmentation is the most commonly performed cosmetic surgical procedure on women in the United States. In 2002, 236,888 women in the U.S. underwent breast augmentation.
According to the National Institute for Women, one in four silicone implant recipients must undergo surgery, within 5 years, to correct implant problems.

Atricle-2-Breast reconstruction
Breast reconstruction is the rebuilding of a breast, usually in women. It involves using existing flesh or prosthetic material to construct a natural-looking breast. Often this even includes the reformation of a natural-looking areola and nipple. This procedure may involve the use of implants.

Overview
Though this procedure was recently out of the question after surgical procedures such as mastectomy, this type of cosmetic surgery is becoming more common. Though most health plans cover the cost for this operation in the US, some do not, considering it be cosmetic; paying for this procedure oneself may be very costly. However, as noted, most plans do cover this procedure as the cost may outweigh the converse cost of counseling for the individual as they try to cope with the emotional adjustment of losing a breast.
For individuals who have undergone a mastectomy due to cancer, they are only eligible for this procedure if the disease was eliminated due to the breast removal. It is possible for this procedure to be carried out immediately following the mastectomy, so the individual awakes with the newly formed breast already in place. As with many other procedures, those with high blood pressure, obese individuals or those who smoke are poor candidates for this operation.
Breast reconstruction is a large undertaking. Most procedures take several operations. Sometimes these follow-up surgeries are spread out over weeks or months. If an implant is used, the individual runs the same risks and complications as those who use them for breast augmentation.

Techniques
There are many methods for breast reconstruction. The two most common are:
- Skin expansion By far the most common method, the surgeon inserts a small balloon expander beneath the skin and periodically, over weeks or months, injects a saline solution to slowly expand the overlaying skin. Once the expander has reached an acceptable size, it may be removed and replaced with a more permanent implant. Reconstruction of the areola and nipple are performed in a separate operation after the skin has stretched to its final size.
- Flap reconstruction The second most common procedure uses tissue from other parts of the patient's body, such as the back, buttocks, thigh or abdomen. This procedure may be performed by leaving the donor tissue connected to the original site to retain its blood supply (the veins are tunnelled beneath the skin surface to the new site) or it may be cut off and new blood supply may be connected.
This procedure has the downside of leaving scar tissue in both the donor and breast area, but, since the donor is the recipient, tissue rejection is not an issue. Also, the patient may end up with a better contoured abdomen if that was the selected donor area.

Follow-up
Recovery from skin expansion is generally faster than with flap reconstruction, but both take three to six weeks to recover from and both require follow-up surgeries in order to construct new areola and nipple. Most scars will not disappear completely, but the better the quality of the reconstruction, the less noticeable and distracting the scars will be. All recipients of these operations should refrain from strenuous sports, overhead lifting and sexual activity during the recovery period (three to six weeks).
